Your Polycom likely supports VLAN tagging. You’ll have to have it tag phone (SIP/RTP) related traffic and configure the connected port on your Cisco switch as a trunk accepting your voice VLAN (and any necessary configuration on uplinks, other switches, routers, etc.)
By “data” I’m assuming you mean the 2nd switch port on the phone is plugged into a computer or other device. If you properly setup the voice VLAN on the phone there’s likely no other configuration to do if you want your “data” to be on the native VLAN of the Cisco.
There’s not enough information to advise fully, but this is the general direction you’ll need to take.
